Important Information

To complete the enrollment process, please provide the following documents:

Birth Certificate
Current year scholarship award verification
Recent health examination (within the past year)
IEP/504 Plan/Disability Verification (if applicable)
Immunization Record or Immunization Waiver (must be current)
Medicaid Card (if applicable)
Most recent grade report or transcript
Social Security Card
Withdrawal Letter/Form
