Start the Enrollment Process
important information
To complete the enrollment process, please provide the following documents:
- Birth Certificate
- Current year scholarship award verification
- Recent health examination (within the past year)
- IEP/504 Plan/Disability Verification (if applicable)
- Immunization Record or Immunization Waiver (must be current)
- Medicaid Card (if applicable)
- Most recent grade report or transcript
- Social Security Card
- Withdrawal Letter/Form
